The word Hentai (変態) literally means perverted in Japanese.
When used in English slang, it generally means Japanese animation or comics with explicit sexual or pornographic content. This usage is not found in Japan, where the word is never used to refer to "normal" sexual content or conduct.
The actual amount of depictions of sexual activity in "hentai" manga or anime may vary. Due to Japanese censorship laws, it is not allowed to depict an exposed vagina, so you often see blurs or bars over the vagina in hentai drawings. Pubic hair is also not allowed, so this rule was circumvented by simply drawing girls with no pubic hair. This has become very prevalent.
